Requirements for application
- Application Fee
- Completed and signed application form or complete an online form
- Copy of child’s birth certificate
- Copy of parent’s/guardians ID documents
- Copy of immunisation/vaccination card
- Copy of child’s resident permit if foreign’
- Signed POPI consent form
- Signed medical consent form’
- Signed Fees structure and declaration of payment form
- Application verification
